1. Age proof certificate, only HSLC or CBSC Admit Card \/ Birth Certificate issued by concerned. District Health Authorities will be accepted.
2. Graduate Certificate or equivalent Certificate for proof of educational qualification.
3. Proficiency Computer skill for 3 (Three) months course for Junior Assistant.
4. 3 (Three) recent self attested passport size photographs.
5. Permanent \/ Temporary Address Proof.
6. Mobile Number with whatsapp facilities.
7. E-mail ID, if any
8.All documents must be produced both in Original and Self attested copies.
9.The applicant must reach the venue by 10:00 AM for enrollment of their names etc.<\/p>\n\n\n\n
